Overview
Social Worker – Medical Social Work in Everett, WA, United States. Start date 06/08/2026 for a 13-week travel assignment. Weekly pay range $1,290–$1,364. Guaranteed hours 72 per 2-week pay period. Shift is 5x8 days. Work setting involves an acute-care environment with a Level II trauma designation and cross-campus coverage between nearby campuses.

Responsibilities
- Conduct psychosocial assessments for adult and geriatric patients and develop discharge plans aligned with patient goals.
- Coordinate discharge planning and care transitions, integrating with social work, case management, and interdisciplinary teams across Med-Surg, ED, and related units.
- Document assessments, care plans, and progress in EPIC; arrange referrals to home health, rehabilitation, or long-term care as appropriate.
- Facilitate resource linkage, including community supports, transportation, and caregiver guidance; advocate for patient and family preferences throughout the care continuum.
- Participate in interdisciplinary rounds and contribute to safe, timely patient discharges and transitions of care.

Requirements
- Licensure: Washington state social work license in-hand or pending accepted at submittal; eligible license types include LICSW, LSWAIC (associate), LASW, LSWAA (associate).
- Certification: BLS required.
- Experience: Minimum 1 year in acute care, discharge planning, and care coordination; strong preference for case management across Med-Surg, LLOS, ED.
- Skills: EPIC charting experience; ability to work in an acute care setting with flexibility to float to like units across campuses.
- Other: Orientation required (40 hours); pre-approved time off allowed (1 shift per posting); weekend coverage every other weekend; holidays may be required (3 of 5).
