Job Description

Position Overview:

The Registered Nurse (RN) for Home Health provides compassionate, patient-centered nursing care in patients' residences. This role involves conducting comprehensive assessments, managing treatments, educating patients and families, and coordinating care with other healthcare providers to promote optimal health outcomes outside of the hospital setting.

  • Start Date: ASAP
  • Contract Duration: 13 Weeks
  • Shift: Mon-Fri 8:30a-5pm
  • Estimated Pay package $1900 (including Stipend)

Key Responsibilities:

  • Conduct patient visits to assess health status, monitor progress, and update care plans.
  • Epic Remote or Home Care Home Base Systems; Wound Vac’s; Wound Care; IV Management; Assess and Teach disease process ie CHF; COPD etc. Catheter insertion and care; PICC line dressing changes.
  • Administer medications and treatments as prescribed, ensuring patient safety.
  • Educate patients and families about disease management, medication adherence, and preventive care.
  • Coordinate with physicians, therapists, and other care team members to ensure continuity of care.
  • Maintain accurate and timely documentation of assessments, interventions, and patient responses.
  • Identify signs of complications or changes in condition and take appropriate action.
  • Support patients' independence and quality of life through personalized care plans and health education.

Requirements

Qualifications:

  • Valid Registered Nurse (RN) license in the state of practice.
  • Minimum of 1 year nursing experience, preferably in home health, community, or outpatient settings.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills for patient and family education.
  • Ability to work independently and manage time effectively in a home setting.
  • Reliable transportation and willingness to travel to patient homes.
  • BLS certification required; additional certifications (e.g., Home Health Care Certification) a plus.
